Our Services: • Support Coordination - Coordinating and facilitating support and services for participants in order to achieve their goals • Plan Management - Overlook clients' funds and liaise with NDIS on clients' plans. • Personal Care - Assistance with daily activities such as bathing, dressing, grooming, and medication management. • Community Access and Transportation - Assisting with grocery shopping, pharmacy visits, and transportation to medical appointments. • Housekeeping - Cleaning services to ensure a safe and hygienic living environment. • Meal Preparation - Preparing nutritious meals and helping with dietary needs. • Clinical Services - This can include skilled nursing care, physical therapy, and other healthcare services that are crucial for individuals with medical needs. • Supported Independent Living (SIL) - 24/7 personalised disability care at our SIL homes allowing you live as independently as possible whilst providing personal care, food preparation, medication assistance, assistance in the community and more for all residents.

I was looking to transfer providers and wasn’t sure where to go. After searching my wish came through and through word of mouth I was referred to My Guardian, who came highly recommended. Since transferring to My Guardian we have had no regrets.

The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

To be eligible for the NDIS, individuals must be under the age of 65, be an Australian citizen or hold a permanent visa, and have a permanent and significant disability that affects their ability to participate in everyday activities.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS can be difficult to navigate, it's important that you prepare and seek out assistance during your application process.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
