We are dedicated to providing 24-hour assistance, ensuring clients, carers, support coordinators, and allied health specialists have access to our services from 6 am to 10 pm every day of the year. This extensive availability allows us to respond swiftly to any changes in client needs, thereby improving the effectiveness of our care. As a registered provider with 11 years of experience under the National Disability Insurance Scheme (NDIS) with a history of 30 years of championing for clients , and also registered with TAC and WorkSafe, IDS ensures all staff meet stringent qualification standards. Every staff member holds at least a Certificate III in Disability or Aged Care or above, and has passed NDIS screening checks, First Aid, and CPR training. Continuous professional development is tailored in collaboration with Support Coordinators and Allied Health Specialists to meet specific client needs. Our Services: Personal Care and Support: • Assistance with daily living activities such as bathing, dressing, and eating. • Toileting and continence management. • Mobility support including transfers, manual handling, and supported exercise. • One-on-one support for personal care needs. Social and Community Support: • Facilitation of engagement in social and community activities. • Assistance with attending events, groups, and community centers. Client Engagement and Planning: We begin our relationship with new clients through a face-to-face meeting to develop a comprehensive support plan. This plan encompasses: • Detailed documentation of client needs, goals, and achievements. • Medication management, disabilities, and support networks. • Allied health specialist involvement. • Risk management and emergency procedures, updated annually in collaboration with clients and support coordinators. Supporting Support Coordinators: Resource Sharing and Information: • Provide detailed service information including eligibility criteria, service availability, and referral processes. • Share resources like brochures and service guides to facilitate a better understanding of our offerings. Collaboration and Communication: • Maintain open communication lines for timely updates on client progress and service changes. • Participate in regular meetings and case conferences to coordinate care plans. • Collaborate with our finance team to manage client expenditures. Training and Professional Development: • Offer training sessions on specific programs and best practices in disability support. • Provide professional development opportunities to enhance the skills and knowledge of support coordinators. Coordinated Care Planning: • Develop and implement individualized support plans in alignment with client goals. • Integrate various services for a holistic approach to client care. Crisis and Emergency Support: • Provide immediate assistance and resources in crisis situations. • Offer guidance on crisis intervention strategies and access to emergency respite services. Feedback and Quality Improvement: • Encourage feedback to improve service delivery and client outcomes. • Implement quality assurance measures and regular reviews to maintain high standards of care. Administrative and Logistical Support: • Assist with scheduling, transportation coordination, and managing documentation. • Facilitate referrals and liaise with other service providers. Outcome Measurement and Reporting: • Collect and share data on service outcomes to evaluate intervention effectiveness. • Provide regular reports on client progress, service utilization, and goal achievement. The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
